Provider Network Expansion Fund

The purpose of the Provider Network Expansion Fund is to support the hiring of PCPs, Specialists, and Advanced Practice Providers that will serve the Medi-Cal and Medicare population in the Inland Empire. Entities that hire qualified candidates are eligible to receive a subsidy up to $100,000 for Advanced Practice Providers, $150,000 for PCPs, and $200,000 for Specialists. All candidates must be a new access point in IEHP's network and not have prior history in the Inland Empire. 

The funding disbursement schedule is as follows: 

PCP - Target 1

 

Threshold of 250 additional assigned members and meets Initial Health Appointment (IHA) Tier 2 (GQP4P). 

PCP - Target 2

 

Threshold of 500 additional assigned members and meets Initial Health Appointment (IHA) Tier 2 (GQP4P). 

 

Specialists - Target 1

 

Threshold of 250 additional member visits. 

Specialists - Target 2

 

Threshold of 500 additional member visits.

APP - Target 1

 

Based on supervising physician meeting Target I. 

APP - Target 2

 

Based on the supervising physician meeting Target II.
