MyPath is IEHP’s palliative care program, providing member- and family-centered care that optimizes quality of life by anticipating, preventing, and treating suffering. MyPath addresses members’ physical, emotional, and spiritual needs while facilitating member autonomy, access to information, and choice. The MyPath program consists of: a palliative care assessment and consultation; advanced care planning; individualized written plan of care; pain and symptom management; mental health and medical social services; care coordination; and a palliative care team.
